What is Cleft Palate Surgery?
The repair of the cleft palate (also known as a palatoplasty) is usually done at six to eighteen months of age. What Causes Cleft Palate?
The causes of cleft palate are not well understood. Studies suggest that a number of genes, as well as environmental factors, such as drugs (including several different anti-seizure drugs), infections, maternal illnesses, maternal smoking and alcohol use and, possibly, deficiency of the B vitamin folic acid may be involved. Cleft palate may occur alone or with other abnormalities that may be hidden or obvious. Up to 13 percent of babies with cleft palate have other birth defects. Some cases involve genetic syndromes, which may pose specific problems for the baby, and may have a high risk of affecting others in the family. For this reason, babies with cleft palate should be thoroughly examined by a doctor soon after birth.
Are there different types of Cleft Palate?
Just as there are variations in the types of cleft lip, there are also different types of cleft palate. The palate forms the roof of the mouth, and extends from the gums just behind the lip to the uvula toward the back of the throat. Cleft palates may be unilateral or bilateral, depending on whether there is a gap on one side or both sides of the midline. The cleft may include just the front of the palate, just the back of the palate, or the entire palate
What problems does it cause having Cleft Palate?
The most immediate problem caused by a cleft palate is likely to be difficulty with feeding. Many babies with a cleft lip can breastfeed. However, some have difficulty in forming a vacuum in order to suck properly. Babies with these problems may need a special teat and bottle that allow milk to be delivered to the back of the throat where it can be swallowed. Sometimes, special dental plates can be used to seal the roof of the mouth to help the baby suckle milk better. Babies who find it difficult to feed may gain weight slowly at first, but have usually caught up by the time they are six months old.
Speech and hearing:
Cleft palate can cause problems with speech. The size of the cleft is not an indicator of how serious such problems are likely to be – even a small cleft can affect speech quite seriously. Most children go on to speak normally after the palate is repaired, although some may develop problems such as nasal speech.
Hearing may be affected because the muscles of the palate affect the ear, making the child more likely to develop “glue ear”. This is a condition where thick sticky fluid accumulates behind the eardrum as a result of an infection of the middle ear. Doctors call it otitis media with effusion. It can cause temporary hearing loss but can be treated with antibiotics or, if it is an ongoing problem, with a minor operation to insert a tiny plastic tube (a grommet) into the eardrum through which the fluid can drain.
Occasionally, cleft palate may also affect the growth of the jaw and the development of the teeth.
Recovering From Cleft Lip Surgery:
Child may be restless for a while after surgery, but doctor can prescribe medication to relieve any discomfort. Elbow restraints may be necessary for a few weeks to prevent baby from rubbing the stitched area. If dressings have been used, they’ll be removed within a day or two, and the stitches will either dissolve or be removed within five days. Doctor will advise child on how to feed child during the first few weeks after surgery. It’s normal for the surgical scar to appear to get bigger and redder for a few weeks after surgery. This will gradually fade, although the scar will never totally disappear. In many children, however, it’s barely noticeable because of the shadows formed by the nose and upper lip.
What does the procedure for the repair of Cleft Palate involve?
Depending on the extent of the gap in the roof of the mouth, cleft palate repair operation will be done accordingly. Surgeon will make an incision on both sides of the cleft, moving the tissue from the sides to the midline thus closing the gap in the roof of the mouth. This attempt at restoration of normal anatomy of mouth would hopefully enable child to eat and speak properly over time. The operation to repair cleft palate may take 2 – 3 hours and child will be required to stay in the hospital for 3 – 4 days.
Recovering From Cleft Palate Surgery:
For a day or two, child will probably feel some soreness and pain, which is easily controlled by medication. During this period, child will not eat or drink as much as usual – so an intravenous line will be used to maintain fluid levels. Elbow restraints may be used to prevent baby from rubbing the repaired area. Doctor will advise on how to feed the child during the first few weeks after surgery. It’s crucial that his advised should be followed on feeding to allow the palate to heal properly.
